First shut off water supply and remove cover on top of handle. They generally just snap in place so you can remove with screwdriver or knife point. Remove screw in center of handle and pull handle off. There is a copper retaining clip that holds cartridge in place. It has a small hole in the center on top that you can insert scratch awl or other pointed tool in and pull straight up to remove. You can try to grasp center stem of cartridge with pliers and pull up to remove. If the cartridge has not been changed for a long time it may be very hard to remove.
You are going to have to replace the entire cartridge so you should have one on hand and you can purchase a tool made especially to remove moen cartridges.Just follow direction that come with cartridge or tool and remove cartridge. I would use the brass cartridge as I have not had good results with the plastic ones. When you get old cartridge out I would use a small amount of faucet grease or petroleum jelly to lube o-rings and then reinstall new cartridge and clip, handle, screw, and cover. Good luck and thank you.

The handle turns past the 'off' point and then drips. This is the Monticello faucets. I can't find pictures that aren't any more than black squares at the Moen site. Have no idea on how to get this...

You need to hold the lever handle off and turn the bell part counter clockwise you may need a small strap wrench or rubber guard that fits on the pliers it was very common for the plastic piece inside the handle hub to break call 1-800-buymoen and they will send you new parts at no charge

ooops, didn't mean for that to be a request, sorry...It was a solution. Although, if you dont want to deal with Moen, you can get one at your local plumbing suplly store. I recommend taking the old one with you (make sure you shut the water off first!). Again, good luck

Bathroom Faucet Handle Fell Off

There is a small allen screw in the base of the handle- the part the stem screws into to turn the water to the on position. If you have lost the one for your handle call moen at 1-800-289-6636 in the US. Their hours are Mon-Fri 8-8 EST and Sat 8-18:30 or follow this link MOEN.
