Normally there are controls for tv on rear to 1 side of tv , different sides for different makes, LG is on right hand side when looking at the screen.

Go to menu, PIP sub menu, go down to Multi Picture then press left.
Another menu will appear, move cursor to NONE then press LEFT.

Closed Caption is turned on. Go into the menu and simply turn it off and this will take care of your problem.

The blue box is the Picture in Picture function.
Press the PIP button on remote till gone.
The No Entry Sign on the inputs is because nothing is connected to these inputs.
Unless I an mistaken you have only a HDMI cable connected.

Sounds like your on the wrong Input, make sure the cable box is turned on and press the Input button on the remote till you get a picture.
