I bought an IPad mini with a passcode and I tried many times to restore to factory and I always got errors like 3194, I believe. Restore failed on several Windows 7 laptops (by putting the iPad mini2 into recovery mode because the IPad mini2 would not go into DFU) that I have and I was finally able to restore to factory using a Windows 10 machine which is just crazy but nonetheless it worked.

If you've got a locked up iPad app or otherwise misbehaving iPad, and
want to reboot / reset your iPad, simply hold the sleep and home
buttons together until the iPad restarts.
You'll see the shutoff slider come up first, but just keep holding.
The screen will go blank and the Apple logo will reappear as the iPad
reboots.
Note: this does not reset any data on your iPad. This is just a restart, equivalent to rebooting your computer.
