The cover is probably the magenetron. That is where the microwaves come from. If the bread was moist, it might have left condensate in the unit which is causing the sparking. Unplug the unit, leave the door open over night. If you can remove that cover, do so. Be sure to carefully replace it before using the unit. Never turn on a microwave with nothing in it.
If you just put the fan blade back on, it will almost certainly fall off again.
The best bet is to slightly rough up the contact area of the shaft with sandpaper and use a little super glue to hold the blade assembly on the shaft, and let it set a few minutes before using it.
I always recommend that you use the hood vent exhaust fan when cooking on the stove top to keep heat, steam, and
grease out of the controls.
There
should be a "mini-manual" (tech sheet) hidden inside the unit
behind the
control panel or hidden on the left side behind the grille, which is
very helpful when troubleshooting, testing, and locating components.

There is a short in the cntrol board. I have seen problems arise like this from mice, spiders and other insects. Power fluctuations can also trick the control into activating. ERIC

Thank you Jerry. Since I do not see any relays shown in the parts manual, would the relay be a part of the PCB Assembly P/N 8169526?

Jerry, I did not realize you added a comment. It is interesting but the microwave seemed to have fixed itself. It no longer runs on it's own. We had just let it stand for a while unplugged, and when I plugged it back in, it was as if nothing had happened. In the end, I just replaced the microwave. I will be taking the unit apart and will report back if I find anything unusual. Thank you for your help. Alex
Jerry, You are correct. When I took apart the microwave, the tall relay had a wire that looked like it turned brown. When I took apart the relay, the contacts were heavily blackened, so they were obviously arching. Thank you.
