1 power the phone off, 
2 press and keep down the keeys green/talk + '3' + '*', - power on the phone at the same time
3 release the keys when the formatting starts.

Just format the phone by pressing * + 3 + green button and switch on the phone. But don't release the three button until it start in normal mode.
