Un plug unit, turn unit over,remove screws on panel covering the belt. ,first start belt on large pulley and then start it on small pulley turn brush by hand to wind the belt on. Google the model number or go to Wal-M and look in their "bags and Belt" book located next to the Vacuum cleaners.
The belt probably broke. Simply purchase another belt and replace. These belts are made for much stress, and if you've had the vacuum for even a month, they can easily break.
It may also have slipped off the motor shaft, causing the brush to spin freely.
Tough one. Typically the only 2 things that the belt contacts is the motor drive spindle and the brush roller. Anything else would only be there to protect the belt from debris. Can you turn the brush by hand and see why the belt is coming off? Is the spindle wobbly? If so, you may need another motor as I don't think it is easy to get parts for the motor. What does it slip off of, the brush or the motor? Are you sure that it is in correctly, as there are usually 2 ways of putting the belt on. If put on correctly, it should not contact any part of the vacuum except the motor shaft and the brush. Also check check there is not a buildup of hair, etc. where the belt goes around the brush. I would try turning the brush by hand and see if anything is interfering with the path of the belt. There is always a chance that some part of your vacuum has broken or malfunctioned and is touching the belt and making it come off. Also, make sure that the brush is correctly installed.
The tension on the belt should be pretty high so that it is hard to pull out of place. If not, you could have the wrong belt. I believe that this takes a type "U" belt if that is any help. Hope this helps and good luck!

Remove the base plate and remove the wheels and axle. Remove the hair and threads that was causing the wheel to lock up. You will need to use a box cutter or similar to remove the debris. Add a couple of drops of light machine oil and reassemble.

This can be because you need to adjust the tension or probably the pulley is no longer aligned. So find out if the shaft is no longer straight also if the belt requires tension.
