Indicates you have installed the clutch plate incorrectly
the raised hub section is hitting on the flywheel bolts instead of facing back into the pressure plate
opening
only fix is box back out and turn the plate around

SOURCE: Removing rotor on a 1991 toyota 4x4 pickup

You have to take apart the 4x4 hub to get to the lock nut for the bearings then you have to reome the hub and rotor assy, then you have to press the wheel studs out to remove the rotor, then reverse the process.

SOURCE: 1991 Toyota pickup 4WD clutch slipping

There is an adjustment for the clutch pedal under the dash where the push rod goes thru the firewall. There is only aprx. 1/2" of adjustment so be careful when making an adjustment, if your clutch is slipping then you need to shorten the throw of the push rod.

SOURCE: I installed a new clutch and now I cannot change

if you have bled the system properly, the only answer is you have fitted the friction plate on backwards.....(the word front, printed,on the metal centre goes toward you/facing you.)check to see if you have full travel on the actuating lever when pedal is fully depressed....

SOURCE: How do i bleed the new clutch slave cylinder for

Just fill the reservoir, open the bleeder on the slave, have someone push all the way down on the pedal, then close the bleeder.
Repeat this until fluid comes out.
Then pump the pedal several times, and hold the pedal down.
Open the bleeder just long enough for the squirt to come out.
Make sure you don't run out of fluid.
When the fluid coming out has no air in it, then stop.

Check the transmission oil. Maybe it is about to change. Some models dont have the shift stick directly on to of the transmission, Some use a linkage from the shift lever to connect to the transmission. If yours is using linkages try checking them. They could be bent or worn off. Usually these linkages have plastic bush at both ends and they wear off with time, making shifting difficult.

Slips out of fith gear

You will need to rebuild the transmission in most cases or replace the shift engagement dogs. You must tear down the transmission and inspect to know exact cause.

Where is the shift linage located on an 1985 toyota 4x4 pickup truck? I still cant shift into gear from thje last support you gave me

Without seeing the problem myself, it sounds like you have a clutch problem. It could be the linkage but sounds like the clutch. A manual transmission's linkage is located on the top. It can be reached from the inside of the vehicle through the floorboard. The linkage is attached to the lower end of the gear shift and enters the top side of the transmission. A vehicle repair manual may be helpful, Haynes and Chilton are good ones. Good luck
