Put the Parking Brake ON.
Use a 6-inch (or larger) "C"-clamp and place the top end of the "C"-clamp on the back of the caliper, but not on the brake hose and not on the brake hose to caliper fitting. Place the screw end of the "C"-clamp on the center of the outside brake pad, but not touching the caliper. Tighten the "C" -clamp forcing the brake pad in, which in fact pushes the caliper piston back into the caliper bore.
DO NOT REMOVE THE BRAKE LINE HOSE UNLESS YOU PLAN ON REPLACING THE CALIPERS!
On the back of the Caliper are two (2) guide pin bolts holding the Caliper over the Rotor. There two (2) types of rear disc brakes on your 2001 Model. But in each case, you have to remove these two (2) guide pin bolts.
On each of these different types of rear disc brakes there are two different type springs. Notice and/or note how these springs are placed before removing. But in each case remove the spring.
Remove the caliper off the Rotor.
Remove the inside Brake Pad out of the caliper. It has three (3) contact spring arms that are seated into the caliper piston, so pry out with a screw driver.
Hang the caliper with a heavy wire (I use a bent wire hangar) and hang the caliper out of the way. DO NOT let the Caliper hang by the brake hose!
With the two (2) types of systems on the rear: 4WD or AWD and 2WD:
IF THERE IS a cotter pin and nut on the axle: remove the cotter pin, and take off the axle nut. If not - do not worry about it.
Once the nut is off, take the Parking Brake OFF: remove the Rotor. On some models, the caliper bracket also has to be taken off before you can remove the Rotor. There are two (2) nuts on the back plate which have to be removed inorder to remove the caliper bracket before removing the Rotor.

If wheel and calipers are off, and no retainers on studs, spray some penetrating lube in center of hub where rotor slides over hub shoulder. Let it sit for a minute or two, then place a block of wood against outside edge of rotor and hit block with a hammer. May have to give it a good hit, around perimeter in a few spots to get it to jar loose in center at hub. Usually just rusted. Better if you have a big rubber mallet, and hit hard all around outside area of rotor, but block of wood will do the job as well.

There should only be two bolts on the back of the caliper holding it into the knuckle. Removing those two bolts should free the caliper. In fact I believe they're 12 or 13 millimeter bolts.
SOURCE: i need to get rear rotor off disc brakes .
spry some of bp blaster or WD-40 on the rotor then use ur hammer and it will work

if you have some mechanical ability you can definitely do this on your own
SOURCE: how to remove rear disc brake pads seems there
There should be two bolts on the backside of the calipers that you have to take off then the caliper will slide right off. Then to take off the break pads you just pop the old pads off and pop the new ones back in. Adding some break grease will help from seizing.
