sounds like a typical ignition coil break down. when they go bad they act up when they get hot. very easy to replace. it is what the plug wires come from.
sounds like a faulty secondary air system or cannister fault.
Check the air cannister and/or charcoal cannister for damage.
Check for damaged or burst air hoses.
Have you by any chance, monitored fuel pressure from start-up to warm-up point where it will not rev? Would not be uncommon for fuel pump to start acting up, about the same time engine gets warmed up.
