P0128 - Coolant Thermostat (Coolant Temperature Below Thermostat Regulating Temperature)
P0410 - Secondary Air Injection System Malfunction
(has to do with exhaust system)
Note: There is a technical service bulletin (TSB) regarding code P0410 for a
number of GM models. The TSB calls for the replacement of the AIR pump and the
installation of a new secondary injection hose.
A p0128 is engine coolant below regulating temp,,,and the p0410 code is comming on from the p0128 code so what you need to do is replace the thermostat and clear the codes and you should be good to go with no lights

The P0455 indicates you have a gross vacuum leak in the EVAP system.
Start at the engine and the vacuum purge valve, start checking the vacuum hoses and lines all the way back to the EVAP canister near the gas tank. Make sure no hose or lines are cracked / disconnected.

I had this problem with my 05 Trailblazer and it happened around 50k miles. The oxygen sensor needs to be replaced and the part costs about 90 bucks but atleast you will have you're check engine light off.
