sounds like the air mix rod is out of adjustment or if vacuum controled, its leaking can you hear a noise in the vehicle or can you find a vacuum hose off in the engine bay hope it gives a little help
The fan is computer controlled and will not come on untill it reaches 212Deg . It should come on though if you turn on the air conditioning, if not you could have a bad fan relay or relay plug.
first thing, have you check the oil level, maybe its low? if the oil level is ok, when was the last time the oil was changed. could be the oil is too old. last thing, possible low oil pressure due to an internal engine problem. (bearing wear)
