I set the temp to225 and it will go all the way to over 600. Sometimes I will set it to 225 and it will drop to 130 like it has shut off? This problem has been going on for the last few times of use

Did you check the fuses, any voltage at the fuses? In the picture, the left side of relay is control, the pcm grounds the control side of relay. Any applicable trouble codes? Check the coolant temp sensor. I'm not saying replace anything. Will the fans come on if you turn on the ac?
The fans should come on around 220 degrees F. Some makes and models, if you unplug the coolant temp sensor, the fan/s may run all the time? The info I looked at was for 1500--5.3L engine--non-hybrid.
Yes, those two parts are different, and "not-related". You have one or two "switches" screwed in to the engine block to turn on your electric fans when the engine is hot., and sometimes some relays also in the wiring.
The PCM is controlling the fans. It is looking at the coolant temp sensor. Once it sees the coolant temp get around 225 degrees it starts turning on the fans.
What temp does the vehicle get to?
Have you compared the reading on the gauge to the actually reading that the PCM is seeing with a scan tool? Many times the vehicle temp will be fine but the gauge will be wrong. Sometimes the opposite.
Temp should be around 200 to 215. The fans will come on at 225 without the ac on. After that engine should cool down to below 225 and the fans will shut off. I suggest that you put a new thermostat in it and make sure the engine is full of coolant with no air trapped in it.
Sounds like like your radiator fan is not turning on when it should.Check fan relay,fan fuse and check for 12 volts at fan motor at connector.If all checks out then fan motor is most likely bad.
U didn't say the engine, if it is a 6cylinder then around 195-200 F, if a V8 they run very hot, around 225 F, so you have a problem, is the coolant clean? no rust present? I would suspect that you need the radiator pulled and roded out by a radiator shop.
