It is called a Blend Door Actuator... located under steering wheel.. have to take the under dash out an its right up in there. I have the same problem. There are 1 or 2 other actuators as well.. they are on the passenger side under dash. Both of my actuators were bad and my blower motor as well. But the actuator is the reason my blower motor went bad. I pit a new blower motor in but it only worked a couple times before it started going in an out due to my driver side actuator not being fixed yet. I see this post is old but I just wanted to answer since I came across the question and you haven't received a proper answer yet. Hope you figured it out!
It's not a blend door actuator , it's called a mode door actuator .
Mode Actuators - Defrost, Panel and Floor
The mode actuators are 5-wire bi-directional electric motors that incorporate a feedback potentiometer. Low reference, 5-volt reference, position signal, and 2 control circuits enable the actuators to operate. The control circuits use either a 0 or 12-volt value to coordinate the actuator movement. When the actuator is at rest, both control circuits have a value of 0 volts. In order to move the actuator, the HVAC control module grounds one of the control circuits while providing the other with 12 volts. The HVAC control module reverses the polarity of the control circuits to move the actuator in the opposite direction. When the actuator shaft rotates, the potentiometer's adjustable contact changes the door position signal between 0-5 volts.
