Trans is shot very common problem you can rebuid it but thats not cheap finding a used one will be very difficult as 96 is a one year only model and there where 2 within that year depending on the buid date of the van

Your transmission need rebuilding. You can buy some TransX and pour it in your transmission. Make sure drive the car to help the TransX to work it way through the transmission then you should get shift reverse. If it does not work then your transmission is badly damage. So you will have to get it rebuild or buy a used one.
your van is going into limp mode! if the computer sees a problem with any of three sensors it will not up shift, speed sensor, input sensor, output sensor, none of these are a major thing

This same thing happened to my daughter,s car, and it ended up being fixed with a transmission speed sensor. This sensor not only runs the speedometer, it helps tell the transmission when to shift too.
Sounds like you need a solenoid pack to start, and possibly a service. If the tranny is not slipping, the solenoid pack is stuck and not tell it to shift up. Could possibly be a TCM, but not as likely.
Hi the Transmission control module is having communication issues with the sitting solenoids that actually preform the shifting once you initiate it by using the in car stick. these solenoids are inside of the transmission pan and you will need to replace them.
you say the fluid looks good... that's great.
now next time your out drive for about ten good minuetes long enough for the transmission to get nice and warm and then pull over to nice level ground and with the vehicle in park check the fluid again. it's kind of hard to read because it always has a lot of bubbles on it and stuff. but it may be low on fluid.. good-luck ray
