have you attempted to clean throttle plate? I would suggest taking hose off of throttle plate.Hold it wide open and spray throttle body cleaner, (with it not running) let it dry if it is really black or gooey spray and wipe goo and fluid off with a rag.put back together. It may not be problem but if air filter was dirty throttle plate most likely is.

SOURCE: Solution for the message "engine fail safe mode" in explorer 2005

I have hearing a lot of this lately with Fords. Most of the time this dash board light comes on due to problems with the Throttle Body (TB)not just your coolant,etc.. If it is sticking, dirty, causing misfires, etc. then this can cause an engine fail safe mode. You might first try putting in some injector cleaner and a higher grade of gas. Many people I know who have had this problem have had to have their TB cleaned and/or replaced. This is something Ford dealerships now about, but hasn't been made a recall or anything. There are service bulletins though.Mainly keeping cleaner in your gas and performing ritual tune-ups/oil changes may be the only thing you can do.Hope this helps

What is the reset fail safe mode on ford Explorer 2005?

This is the best answer I could find but it appears that your owners manual has the answer in it unless you are in the position this person is in after disconnecting battery. Good luck.

1995 Ford Explorer ran fine, then after night of -20 F weather, won't start. Have put a new battery in. Didn't help. Could it be the fuel pump?

But you don't know what actually froze, cars are not designed for these temperatures, the ones that are use different oils and even fuel, there is even the possibility that your starter motor is still frozen, I have seen news stories of cars turned into ice sculptures and fountains turned into Ice in Texas, where I live I am happy our day temps are under +35C. Having codes P0306 and P0316 show up for 2005 Ford Taurus

Well code 306 is for a miss on cylinder six and code 316 is for a miss detected on startup, which you probably already know.
Since I assume you have replaced the ignition parts for that cylinder,
the next component to check is the injector. You may want to check fuel pressure too.

Reading codes

Check the wiring to the throttle body, check the supply voltages as well. Check fuses too, it sounds like it received a voltage spike and then lost voltage and is now forcing limited rpm.

Is the map sensor disconnected? Check your intake tube for something that is not right, it sounds like something basic.

Engine starves for fuel after running about 50 miles or so...

Has Neon do same, I ran fuel at low level, on hot day, ran engine until it started to act up & threw a fuel pressure gauge on it. Was at approx 5 psi. Cold next morning, 48-50 psi. Always hot day about 30-40 miles into work in morning, o.k. Sit in hot sun all day & would happen 3-4 times on way home. Sit and let cool, 20 minutes or so & go again for a while. Cool day, no problem. Change pump I would.

Try having one person turn the key to the start position and hold it there, while someone else taps on the starter with a hammer. If tapping on it makes it work, you need a starter. Good luck. Hope this helps.
