Fan only blows on high speed does not work on low speeds.
Standard problem with GM, Blower Resistor needs to be cleaned or replaced. This unit is usually on the blower housing

It is a blower resistor set. This resistor set regulates current which gives you the various fan speed settings. Depending on the vehicle it is located under the dash on either passenger or driver's side.

If it was the motor that went out then it would not blow at all. The resistor is the culprit that starts killing off speeds. It should be about $25 to $30 at an aftermarket store like autozone or advanced auto parts.

You have a bad blower motor resistor. This is located under the dash close to the blower motor itself
