Heat seems fine but blower motor won't come on sometimes. When it does work it works on all settings (high to low). Sometimes when you hit a bump, it will come on.

We have the same problem again with our bonneville. The first time the blower motor had worn out its brushes. Now, the connector is a bit melted. I think either the blower motors are cheap, or the mouting position is causing problems (probably a little of both). A few months ago is when the 2nd round started. I pulled the blower and the thing had a lot of carbon dust in it. I cleaned it with contact cleaner and that worked (the brushes still had life in them as they should being less than a year old). So look at you connector and the blower motor first. I would bet that you can slightly manipulate the connector and it will go. Almost forgot, the connector on our bonneville had no weatherpak in it which doesn't help keep the connector firmly in the motor's recepticle. We have the same problem again with our bonneville. Since you eliminated the relay and blower resistor as possible problems, the only thing left is an intermittent blower motor. Have had the problem before and when the motor was replaced, the problem was gone.
I would start with the rear blower motor resistor, the resistor is what controls the speed of the fan, the resistor is located in the rear heater box near the blower, you will need to remove the right rear side panel to access the heater box. start with that
I can't diagnose unless you can do some testing? This is part of diagram for automatic hvac. At the blower motor, terminal B is voltage, terminal A is ground. Use a test light, check voltage and ground with hvac on, Have to check when problem is ongoing. If good voltage and ground with no blower action, suspect the blower motor is faulty. Of course, if your missing voltage or ground, have to track that circuit. Faulty blower motors may run intermittently. When problem is ongoing, hvac on, you can try tapping blower motor with screwdriver handle see if it starts running, if it does, I suspect the blower motor has a problem. Check hvac trouble codes, where applicable.
The temperature display and the heater function are likely two different issues. I had a similar issue with intermittent heat on my 2001 Aurora. In my case it turned out to be the blower motor. As a vehicle ages and the bearings on the motor start going bad, the amount of energy it takes to spin the motor gets higher and higher. This can result in intermittent behavior or cause the heater resistors to go bad. It is also possible that the heater control itself is the issue, but with the age of this vehicle, I would start with the blower motor and then heater resistors.
With the vehicle running and the blower switch set to high and the blower motor not working tap the bottom of the blower motor with the handle end of a screwdriver. If the blower starts to work it is bad. Usually the brushes are worn out and tapping the blower wiggles them just enough to make contact with the commutator and the blower will work.
I would check to make sur the engine coolant is filled up to the required level. Sometimes if it is low it wont be enough to circulate through the heater core. As for the heater blower, thats a whole other issue. There is a resistor pack which may be the issue. If so it should work on high. Or maybe the blower motor has a bad spot on the armiture. That could cause an intermittant issue. Beyond that there could be some other electrical demon in there. But so long as you get the hot water circulating there should be some heat.
Check the blower resistor pack - usually located in the heater air ducting in the blower fan airflow. Check for loose connections and open circuit resistors on the unit.
If speed 4 works always, its unlikely to be the blower motor ground connection.
Remove the hush panel under the dash and unplug your blower motor turn the knob and see if its getting power. If it is replace the blower motor. If your heater seems to work on high but not the other 4 settings. replace the blower motor resistor and pigtail (same general location as blower motor)
