Transmission rebuilt about 5 months ago.
Hi there:
I would suspect it is the shifter not the cable or the transmission to rule out the trans from under the explorer on the drivers side near the pan is the shift linkage disconnect it by prying the ball and socket apart and rotate the lever through the gears to the rear is park then one forward is reverse then N,D,3,2,1 when pushing the lever into park it will feel spring loaded if the parking pawl is not perfectly lined up this will line up if it rolls a little bit DANGER DO NOT LET VEHICLE ROLL OVER YOU.

Check engine light on? Sounds like a bad pressure control solenoid. If it is a bad PCS. You have to replace the solenoid block in the 5R55W transmission.

SOURCE: 98 Explorer timing chain?
If this is a SOHC 4.0 V6 then there is a recall on the tensioners for the timing chain, they cause the kind of noise you speak of, check with ford, this is the SOHC only.
SOURCE: transmission shifting problems
If Drive Selector switch is properly aligned then a shift silonoid might be the problem.
SOURCE: 1992 Ford Explorer transmission will not go in
No reverse is almost always an internal problem (no external fix), and is usually a problem with the reverse drive shell, such as stripped splines.
