1: Change your fuel filters and bleed the air out of it.
2: Check all rubber hoses for loose hose clamps and
cracks or leaks.
Almost certainly in one or both of the two points above.

ONe thing to check is that your throttle position sensor (TPS) is working as it should and that the electrical connections to it are clean and tight and that wires associated with it are intact. When your TPS starts misbehaving by sending the wrong position values to the ECU then erratic engine revs and associated difficulty in gear changes follow. TPS is a an electrical sensor mounted on the outside of the throttle body on the end of the throttle plate shaft. As the throttle is opened the shaft rotates. The amount of throttle (shaft rotation) is measured by a contact within the TPS sliding on a carbon track. The TPS has three electrical lines to it. One is at consistent low voltage ('throttle closed' end of the track) the other is about 5 volts ('throttle fully open' end of the track) and the third is the variable sliding contact which sends a voltage corresponding to the throttle position to the ECU. The TPS is pretty easy to check out with a voltmeter and you can read the variable out put by simply turning the throttle shaft by pulling on the accelerator cable. If the TPS needs replacing it is a pretty cheap and easy to do. Hope that helps some
