I had exactly the same problem on my UK 2001 S60.
It was caused by the throttle control housing gaskets not being serviced properly and consequently the entire housing had to be replaced - not a cheap job -and a valuable lesson learned - use a reputable volvo specialist to service your volvo - I didn't and I learnt the hard why! only a volvo specilaist would have known about this I went to several other main street garages & none of them could even diagnose the problem let alone fix it. Although they did do numerous other things first before they gave up - ie replaced titanium spark plugs.
Before it stoppped running altogther and went into "limp home mode" there were tell-tale engine/throttle surges when the car was idle, but in gear.
It is now running "like new" & I will never get another garage to service it, as this could have been avoided - however they can also wear out after about 10years - so my bad services just made it happen sooner!

In the past we have used starting fluid to find vacuum leaks. Start the engine, let it idle. Spray quick shots of starting fluid at your vacuum lines in isolated areas. When the starting fluid find the vacuum leak the engine will react to it... usually by going up in rpm. Depending on your engine computer, it may stall the engine. Either way you will find your vacuum leak. Get starting fluid or "ether" at Autozone or O'Reilly.

DEFINITION: A large engine RPM increase is noted in fourth gear, Drive range, with little or no vehicle forward speed increase. First, second, and third gear operation are normal. Shifts into fourth gear are soft or do not occur. Fourth gear ratio is 0.705:1.

Oil Filter Assembly
• The oil filter assembly (100) is missing, plugged, or damaged.
• The oil filter seal assembly (101) is missing or is damaged.
Control Valve Body Assembly
• The 3-4 shift valve (362) or the 4-3 manual downshift valve (360) is stuck or binding.
• Debris is found in the passages of the control valve body (301) or of the control valve body spacer plate and gasket assembly (396)
Case Cover Assembly
Distortion or damage occurs at the manual valve (404) or at the case cover (401), causing PRN oil leak.
Driven Sprocket Support Assembly
A plugged third clutch oil passage in the driven sprocket support assembly (609)
Fourth Clutch Assembly
• The clutch plates (500-502) are worn, damaged, or misassembled.
• A missing or not seated fourth clutch spring retaining ring (601)
• A damaged spring assembly (602) or piston (603)
• Worn or damaged fourth clutch piston seals (604, 605)
Third Clutch Sprag Assembly
• The sprag (720) was misassembled or backwards on the inner race (661).
